Job Description:

This position analyzes inpatient and discharged accounts by identifying portions of information to complete the medical record.  Analyzes medical record content for presence, accuracy, completeness, timeliness, clarity, authentication, and legibility, in accordance with The Joint Commission, State Dept. of Health, and CMS requirements.  Other responsibilities include notification of physician regarding deficiency, assisting physician with deficiency completion, tracking deficiencies per physician, reporting deficiencies to the HIM Director, Medical Staff Services Dept. and Medical Staff Committees, as appropriate.


Associate degree in Health Information Management with successful completion of the Accreditation Exam

Active membership in AHIMA or equivalent with experience in Health Information Management

Minimum of one year experience in a medical record department with familiarity in working with diverse facets of medical record activities

Ability to work effectively with other departments, administration, physicians and all ancillary staff

Familiar with federal and state laws and regulations concerning medical records and TJC standards

Demonstrates knowledge of the electronic health record in all applications that apply to the patient health record.

Demonstrates knowledge of Sunrise Clinical Manager (SCM) and Hyland Onbase and assigns deficiencies with 98% accuracy.
