Job Description:

RN, Care Manager

Oncology Clinic at Home

**This position is not remote, it requires the ability to work onsite at CTCA Atlanta**

Mon - Fri; 1st/Days

Oncology Clinic at Home is an innovative program that utilizes an integrated approach to safely provide chemotherapy in the home with the use of digital therapeutics and coordination of care with the care team. Based in the clinic, this role will serve as the clinical point of contact for Oncology Clinic at Home working directly with the care team. Responsibilities include

  • Coordinating patients from initial consultation and onboarding through treatment at home and follow up

  • Collaborating with the patient, the clinical care team and caregivers for coordination of care

  • Communicating with providers and ensure patient treatment requirements are met

  • Assisting patients in navigating any barriers to their care

  • Responding to patient reported concerns and support with symptom management using clinically relevant, evidence-based plan for escalating concerns and symptoms to care team and providers

  • Assisting patients with intake applications and forms

  • Facilitating patient onboarding and education

  • Creating and maintaining patient files and ensure appropriate documentation in patient charts

  • Communication with external organizations on patient’s behalf and serve as liaison

  • Providing clinical assessment services and referral sources, as needed.

Required Education/Experience

  • Current Georgia RN license

  • BSN required or enrolled within 6 months of hire and BSN completed within three years from hire date

  • BSN in Nursing or Case Management certification preferred

  • 3 years clinical oncology experience preferred

  • 1 year experience in Case Management and/or similar role  

  • Home health nursing experience, a plus

  • Project Management experience, a plus

  • OCN certification preferred

  • BLS certification required
