Coordinates the new patient process and referral/transfer of patients to the hospital setting. Provides clinical assessment services to referral sources, families and patients to determine the appropriate level of care needed for potential services within Cancer Treatment Centers of America. Uses evidence based clinical knowledge to ascertain appropriateness of patient for evaluation and treatment at Cancer Treatment Centers of America. Assesses special travel and medical needs while conducting interviews and makes sure schedulers or patient navigator are kept apprised of these needs. Works very closely with other team members in an effort to provide the most integrative model of care to our patients. Reviews new chart created by OIS and Patient Navigator, abstracts chart and ensures that all necessary items are in place for appointment with physician. Responsible for the initial in person health assessment, medication review with patient, obtaining vital signs and entering all information into electronic health record.
Key Position Qualifications
- Current state RN license from an accredited college required
- BSN required or enrolled in a BSN program within 6 months of hire and completed within 3 years of date of hire
- BLS/CPR certification required
- 3 years clinical oncology experience preferred
- 1 year experience in Case Management and/or similar role preferred
- Prior RN experience in admissions or intake preferred
