CANCER TREATMENT CENTERS OF AMERICA (CTCA)Cancer Treatment Centers of America (CTCA) is a national network of regional, destination hospitals specializing in complex and advanced-stage cancer care. CTCA's newest hospital is located in Newnan, Georgia which is located in the Metro Atlanta region. It is a 226,000-square-foot, all digital facility focused exclusively on treating cancer. It includes 50 beds, ICU rooms, surgical suites, state-of-the-art radiation and infusion therapy departments, outpatient clinic rehabilitation and physical therapy department and onsite residential accommodations for outpatients and their families. We are currently seeking dedicated Stakeholders to help us fulfill our Mission and provide the Mother Standard of Care® to patients in the Southeast.
To demonstrate support of the hospital mission and vision by providing case management/disease management services to clients and families to help in the coordination and management of individuals' health needs through the full continuum. The care manager demonstrates the ability to assess, coordinate, plan, monitor, implement and evaluate the patient's plan of care while ensuring and facilitating the achievement of optimal quality, clinical and financial outcomes.
Provides disease management and, understanding oncology, chemotherapy and the side effects, is able to monitor and address symptoms. Works with patients and families throughout the continuum of care to provide an understanding of their disease, their treatment plans, and address patient quality of life issues. Symptom management calls are made proactively to anticipate needs of patients undergoing treatment and to manage side effects of disease and/or treatment. Documents assessment, toxicity screen, and interventions entered in a timely manner in the electronic medical record. Collaborates with Oncologists daily to assist in managing and coordinating patient care. Effectively and efficiently triages patient medical issues either in person or telephonically. Intervenes appropriately with use of standing orders and/or consultation with the interdisciplinary health care team. Rotates being on call nights, weekends, and holidays and returns pages in a courteous, professional demeanor in a timely manner.Meets with patients on their first visit, conducts admission history. Discusses and educates patients and families about Care Management services. Meets patients again at follow-up appointment to ensure patient has all information to make informed decisions regarding treatment, understands the role of Care Management, and how to access services appropriately. Documents timely and thoroughly in the electronic medical record.
